Indemnity Agreement and Waiver of Liability I, the undersigned, hereby acknowledge that participation in Taekwondo classes, events, and related activities (“Activities”)
at Global Taekwondo Academy involves inherent risks, including but not limited to personal injury, property damage, and other risks associated with physical training and martial arts practice. In consideration of being permitted to participate in any way in the Activities conducted by Global Taekwondo Academy, I hereby agree as follows: Assumption of Risk: I voluntarily assume full responsibility for all risks of injury, loss, or damage to person or property that may be sustained in connection with participation in the Activities. Waiver and Release: I hereby release, waive, discharge, and covenant not to sue Global Taekwondo Academy, its owners, instructors, staff, affiliates, successors, and assigns from any and all claims, demands, actions, or causes of action arising out of or related to any loss, damage, or injury, including death, that may be sustained while participating in the Activities. Indemnity: I agree to indemnify and hold harmless Global Taekwondo Academy and its representatives from any and all liability, loss, or damage, including attorney’s fees and costs, incurred as a result of any claims or causes of action brought by any person arising out of my participation (or my child’s participation, if applicable) in the Activities. Medical Emergency: In the event of a medical emergency, I authorize Global Taekwondo Academy to seek medical treatment and agree that I am financially responsible for any and all medical services rendered. Acknowledgment of Understanding: I have read this indemnity and waiver of liability clause and fully understand its terms. I understand that I am giving up substantial rights, including my right to sue. I acknowledge that I am signing this agreement freely and voluntarily.I agree to the terms of the Indemnity Agreement and Waiver of Liability.
